| These rules apply to the feedback prize drawing conducted by Elijah Tooling, Inc. from December 12, 2009 - January 31, 2010 Prizes FIRST PRIZE (1 winner): 10 of our newest fasteners + insertion tool ($400 value) SECOND PRIZE (2 winners): 5 of our newest fasteners + insertion tool ($234.70 value) THIRD PRIZE (5 winners): $10 Starbucks gift cards How to Enter Entrants must be 18 years of age or older as of December 12, 2009. Your name will be entered into the drawing once you complete the feedback form with positive or negative feedback. Your name, phone number, physical address, email address, and comments are required. You do not need to write a comment for each section. You are limited to one form submission per person but persons in the same office or place of business may each submit one form. How to Win Winners will be randomly drawn from the pool of those who complete the feedback form. The drawing will take place on January 31, 2010. Winners will be notified via phone, email, and website. The Fine Print NO PURCHASE IS NECESSARY OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER AND/OR WIN THE DRAWING.
